When this tissue is irritated or inflamed, it creates pain typically at the heel where the fascia attaches to the calcaneus (heel bone).
Classic Plantar Fasciitis Symptoms:
Pain primarily at the heel, especially at the bottom-center
Pain worst with first steps in the morning or after prolonged sitting
Pain that improves with activity and gradually worsens as you continue
Pain that shoots or radiates from heel toward arch

Heel Bruise (Heel Contusion)
A heel bruise is trauma or irritation to the fatty tissue padding the heel. It creates sharp pain in the heel that can feel very similar to plantar fasciitis.
Key differences: heel bruises usually develop acutely (sudden onset after an impact or change in running), are extremely tender to direct pressure on the heel, may have visible swelling or bruising, and often respond quickly to activity modification and padding.
Plantar Fat Pad Inflammation
The heel has a fatty pad that acts as shock absorption. When inflamed (from overuse, impact, or training changes), it creates heel pain.
Key differences: pain is centered in the heel, worsens with impact activities, responds well to padding and shock absorption, and often develops with sudden training load increases.
Heel Stress Fracture
A stress fracture of the calcaneus (heel bone) creates heel pain that can feel like plantar fasciitis but is more serious.
Key differences: stress fractures create severe, persistent pain that often worsens progressively, don't improve with conservative treatment, require imaging to diagnose, and may require significant activity restriction.
Achilles Tendinopathy
Achilles tendon inflammation creates pain at the back of the heel, which is sometimes confused with plantar fasciitis (which is bottom of heel).
Key differences: pain is at the back/top of heel where Achilles attaches, worsens with heel raises, improves with Achilles stretching and strengthening, and has different mechanics than plantar fasciitis.
Baxter's Nerve Entrapment
Baxter's nerve can become compressed, creating pain in the heel and foot.
Key differences: pain may be more lateral (to the side) than classic plantar fasciitis, symptoms progress with activity, and stretching/strengthening sometimes worsen symptoms initially.

History and Symptom Evaluation
Detailed questioning about pain onset (sudden vs gradual), progression (improving, stable, worsening), specific location, what makes it better or worse, and previous incidents identifies patterns unique to each condition.

Plantar fasciitis requires addressing the inflamed fascia through: progressive stretching and strengthening (especially calf and arch), activity modification (reducing high-impact activity temporarily), appropriate footwear, arch support or orthotic intervention, anti-inflammatory measures when appropriate, and gradual return to running as symptoms improve.
Most plantar fasciitis cases improve significantly within 4-8 weeks with consistent treatment.
Heel Bruise Treatment
Heel bruise treatment focuses on: protecting the heel from impact through padding or heel cups, activity modification (reducing running volume temporarily), ice and anti-inflammatory measures, gradual return to activity as pain improves.
Most heel bruises improve within 2-4 weeks with appropriate protection and activity modification.
Heel Stress Fracture Treatment
Stress fractures require: significant activity restriction or immobilization depending on severity, imaging confirmation, medical physician involvement, extended recovery period (often 6-12 weeks), and gradual return to impact activity only after medical clearance.
Achilles Tendinopathy Treatment
Achilles issues require: eccentric strengthening exercises, progressive loading, activity modification, stretching, and gradual return to activity.
Most Achilles issues improve within 4-8 weeks with consistent eccentric training.
Preventing Heel Pain When Running
Prevention strategies differ based on what you're trying to prevent.
For Plantar Fasciitis Prevention:
Adequate calf flexibility and strengthening
Appropriate arch support
Gradual training load increases
